In collaboration with the Director of Stroke Division, provides leadership and facilitation to promote improved neurological patient outcomes. The Stroke Program Coordinator serves as expert clinical liaison among the Departments of Neurology, Neurosurgery, Medicine, Emergency Medicine and Nursing as it pertains to the care of stroke patients and stroke survivors. Serves as role model, educator and resource person to facilitate professional development of health team and increase community awareness. Creates ongoing patient education specifically tailored to the patients and families. Assures adherence to educational protocols and clinical guidelines. Active involvement in data collection and Performance Improvement is required. Meets the qualifications to provide care for patients in the specific age range on their assigned clinical area.
The Stroke Coordinator will play a primary role in maintaining Gold Plus Joint Commission status as a Primary Stroke Center. Coordinates the Joint Commission Stroke Program and American Heart Association Get with the Guidelines programs, ensuring that all quality metrics are measured, rapid implementation of quality improvement process when they are not, and maintenance of all accreditation guidelines.
Assists in the clinical evaluation of patients. Responds to stroke alerts. Assists with management of patients in the Neurology hospital discharge clinic. Assists with the Neurology tele-medicine program.
Performs duties to support the goals and objectives of the Geisinger Health System in its efforts to provide quality patient care and patient satisfaction.
Reports directly to the System Stroke Coordinator.

EDUCATION AND/OR EXPERIENCE:
Graduated from an approved School of Nursing required. BSN required.
Requires registered nurse currently licensed in Pennsylvania or eligible to be licensed.
Must participate in continuing education activities to maintain and enhance knowledge.
Minimum of four- five years of clinical experience required, preferably in a neurological related field.
Must be currently licensed in BLS and hold an updated CPR card.
Experience in stroke management a plus.
Strong analytical, investigative skills, program/protocol development and implementation experience required.
BLS certification required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in a clinical inpatient setting.
