San Ramon Regional Medical Center began serving residents of the San Ramon Valley and its surrounding communities in 1990. Located on a hillside overlooking the valley, we are a 123-bed, acute-care hospital, primary stroke center, and a cardiac heart surgery hospital. Reporting to the Director of Clinical Quality Improvement, the Patient Safety Officer/Risk Manager plans, organizes and coordinates all aspects of the Risk Management/Patient Safety Plan within the facility. The role holds overall accountability for developing/maintaining an evolving and comprehensive risk management program including clinical risk identification, risk evaluation and implementation of corrective action related to identify risk issues. Assesses, coordinates, and implements multidisciplinary efforts to insure and maintain the safety of the hospital's patient population This position has the ability to work at a fast pace and to prioritize multiple assignments/projects and respond to numerous requests and the ability to resolve conflict and to work collaboratively with administration, department directors and managers, staff and other members of the healthcare team. The role requires thorough knowledge of regulatory/accrediting agencies, statues, and reporting requirements as well as claims management reporting requirements. The position will provide direct leadership for safety assessments and coordinate the activities of the Patient Safety Committee within the organization. He/she educates other practitioners on the system-based cause for medical errors, consults with management and staff, and communicates literature-based ideas regarding effective patient safety strategies to others within the organization. Key responsibilities of this position also include working with all services to hardwire reliable delivery of healthcare systems that incorporate best practice patient safety methodologies and will lead initiatives to improve the reliability of health services delivered at the point of care. This position will interact with all levels of the organization through education and reporting on the status of the organization wide patient safety program. This position is responsible the development of a comprehensive program for reducing clinical risks at the point of care and mitigating those risks once they occur in collaboration with their Regional Claims Manager and Tenet Risk Management Department. He/she will assist in planning, implementing, and evaluating the effectiveness of its risk prevention program through tracking of adverse events and through developing and reporting of other key risk management metrics. He/she will assist in collaborating with the facility Quality Management function in managing compliance with all applicable federal, state, local regulations i.e. (EMTALA, FDA reporting, etc.) and will work closely with the facility Hospital Safety Officer and Hospital Compliance Officer in managing the Quality of Care domain of the company wide corporate compliance program. Duties will include but are not limited to: Managing the Occurrence Reporting System (ESRM), managing claims, peer review: performs review of medical staff clinical indicators reviews charts identified from a variety of sources (medical record, EPF, HIM, reports, etc.) and identify cases needed for referral. Performs review of Hospital occurrence reports pertinent to medical staff; determine which assess need further follow up or review and refer in timely manner. Investigating sentinel events and developing RCAs, presenting risk/loss, assisting with litigation management, communicating with Corporate Legal Department, preparing reports for MEC/Governing Board, participating in disaster planning, managing complaints and grievances, maintaining knowledge of regulatory/legal requirements and reporting discrepancies when required, processing and reporting incidents that fall under the federal regulation known as Adverse Event Reporting and the Safe Medical Devices Act, providing information and statistics regarding risk management issues and process, advising administration and staff on risk prevention activities and administrative program changes that may have risk management implications.
Minimum Education: Bachelors Degree required; Masters Degree preferred in a clinical or health care related field; Certification in Risk Management preferred at time of hire or within 24 months of accepting the role. CPHQ or CPHRM or National Patient Safety Foundation, Certified Professional in Patient Safety - eligibility to sit for exam within two (2) years of taking position.
Minimum Experience/Skills: Five (5) years experience in an acute care hospital setting required, experience and understanding of hospital risk and/or claims management, working knowledge and experience with Root Cause Analysis development, ability to interpret medical records to identify potential liability, must be able to interpret legal contracts, excellent communication and presentation skills, ability to lead and coordinate activities and resolve conflict within a cross functional diverse group of people, ability to work at a fast pace and to prioritize multiple assignments/projects and respond to numerous requests, must be proficient in the use of Word, Excel, PowerPoint, and various other computer programs.
