Responsible for data collection and process improvement for patients throughout the hospital with special emphasis on quality improvement for Stroke Services. The Coordinator will coordinate the efforts between providers, hospital administration, and staff while tracking the progress of the Stroke program and identifying areas requiring attention. The Coordinator will ensure compliance with all relevant regulatory, accrediting and legal authorities within the Stroke program.

Interrelationships: Works closely with 4C/9C unit director, ICU/CCU unit director, ED Director,and staff. Frequently interacts with Stroke Medical Director. Frequent interaction with neurologists, physicians, Nursing Departments, and other hospital departments. Frequent interaction with patients and patient’s families and the public.
Responsibilities and Authorities:
Performs other duties as assigned by the department's management staff.
As patient care and/or Hospital need dictates, both the assigned shift and work station may be changed by the department.
Participates in and promotes the “I CARE” service philosophy of Integrity, Customer Centered, Accountability, Respect and Excellence.

Reviews charts and medical records for all stroke patients. Recommends quality improvement for patient outcomes and the department.
Makes patient visits and real time chart reviews for stroke patients. Works closely with attending physicians and neurologists, and neurosurgeons to ensure evidence based care.
Serves as clinical expert for stroke patients and recommends outcome improvements through appropriate channels.
Assists in development of clinical pathways, protocols, order sets, and guidelines for stroke patients.
Reviews stroke data and meets all applicable deadlines.
Facilitates implementation protocols, P&P related to Stroke and educates/communicates to staff and physicians.
Serves on Regional Advisory Council and on EMS QA Committee.
Disclose all information requested by administration in a timely and accurate fashion.M. Lead regularly scheduled meetings with hospital administrators, Provide quarterly Stroke education to Nursing units, medical directors, other physicians, and key staff.
Performance Factors Used to Evaluate Employee:
Coordination of Stroke data collection and analysis.
Assessment of patient acuity and outcomes. Reviews charts and medical records.
Education: Currently licensed as a registered nurse in the State of Texas. BSN preferred.
Training and Experience: A minimum of three (3) years clinical experience required. Acute care experience required particularly in the acute care of Stroke patients.
Job Knowledge: Demonstrated statistical and analytical skills; knowledge of cardiovascular physiology and disease. Possesses ability to assess data reflective of the patient status and acuity. BLS and ACLS knowledge required, certification preferred. Computer, verbal and written skills required: self-motivated, goal oriented and ability to tolerate frequent interruptions.
