Position will be located at the Orlando VA Medical Center - Lakemont Campus. Nursing Case Management is a dynamic and systematic collaborative approach with internal and external customers to provide and coordinate health care services to a defined population to meet the individual's whole wellness experience. It is a participative process to identify and facilitate options and services for meeting the Veteran's health needs, while decreasing fragmentation and duplication of care, and enhancing quality, cost-effective clinical outcomes. The framework for nursing case management includes five components: assessment, planning, implementation, evaluation and interaction. Areas of Practice: Behavioral Health, Suicide Prevention, Housing Urban Development Veterans Affairs Supportive Housing (HUDVASH), Poly-trauma-Traumatic Brain Injury (TBI), Surgery, Mental Health Intensive Case Management (MHICM), Integrated Health Service (IHS), Operation Enduring Freedom(OEF)/ Operation Iraqi Freedom (OIF)/Operation New Dawn (ONO), Medicine Specialty Areas. Work Schedule: Monday-Friday/0800-1630. *Workday/hour flexibility required as administrative and/or clinical demands require. Financial Disclosure Report: Not required.
