The Erie VAMC is currently recruiting for a full time Registered Nurse to serve as the Specialty Care Clinical RN and will work as an extension of specialty care services at the Western Market (WM) facilities and satellite offices. A portion of their duties will be allocated to the Referral Coordination Initiative (RCI) in efforts to retain veteran's workload for the specialty service trough the RCI processes. The Registered Nurse will demonstrate leadership in delivering and improving care through collaborative strategies with specialty service team members locally and within other WM facilities. They will work in partnership with patients, team members and programs leadership for effective disease/chronic care and program management and development. Specialty care services the incumbent may be dedicated to can be but not limited to Rheumatology, Endocrinology, Cardiology, Neurology, Orthopedics, and more. They will be the onsite point of contact for referral coordination, hands on care, and virtual assistance for the veteran and providers. They will strengthen the provider patient relationship at a facility level. They will manage clinics, workflow, access issues, RCI, CVT assistance, patient education and medication treatments, etc.. They may be required to travel to outside facilities associated with their duty station. The Incumbent will assure that all patient care needs are met in a safe, efficient, and timely manner that meets acceptable standards of care and expectations within the Specialty care service line as well as the Referral Coordination team model of care. The Incumbent will continuously evaluate processes and identify opportunities for improvement and utilizing system redesign principles to improve care and services. Is responsible for quality and performance measures in the clinic and stays current and informed of VA, Joint Commission, National, State, and local standards, regulations and requirements. The RN provides input for Medical center, program, and clinic policies and procedures and provides leadership in identifying and addressing ethical issues that impact patients and staff. The Incumbent will ensure excellent customer service and will act as the first line patient advocate in response to any complain or concerns with the clinic. This position may include other clinical or administrative assignments necessary to meet the mission of the VA Health Care System as well as cover other clinic locations at the discretion of the WM and Chief of Specialty service lines. The RN reports directly to the Nurse care manager for specialty care services at the facility. The RN will work as a member of the health care team that will promote and maintain health by providing the following medical services: Obtaining medical histories through chart review, telephone histories, and virtual visits, with occasional travel to WM sites for in person physical exams assistance in conjunction with specialty care provider. Patient education on medications, disease processes, and other pertinent healthcare maintenance. Review consult needs and anticipates testing requirements needed for the identified specialty care service line. Participate in necessary clinical coordination between services and outside communities. Knowledge and skills necessary to carry out more specialized patient care duties, including assignments in selected, more complex care settings. Knowledge base promotes the incorporation of current patient care data in actively contributing to the development of the patient's overall health plan and in carrying out assigned nursing interventions. Positions at this grade level require broader knowledge and skills and are expected to be involved to a greater degree in the development and modification of treatment plans, maturation of clinical programs, and pilot success. They will actively participate in clinical planning through TEAMS meetings with specialty clinic providers and pharmacists as directed to develop treatment plans and appropriate clinical care.
