Assumes a supportive role and has responsibility and accountability for facilitating, communicating and collaborating with the healthcare team and the patient/ family, utilizes best practice to maximize patient engagement and patient selfmanagement efforts. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. The LPN is expected to identify normal from abnormal in each body system and to identify changes in the patient's condition which are then reported to an RN or LIP for further assessment, treatment and follow-up. Provides direct patient care that is consistent with the core values & standard behaviors laid out by this institution. May perform a variety of clinical tasks as directed by an authorized health care provider. May assist with front office duties and scheduling of tests and/or patient appointments. Responsible for accurate chart documentation and medical record maintenance. Attends / conducts in-service presentations and seminars to facilitate personal and professional growth. May involve some project work and other office duties as it relates to the department(s) business needs. Other duties as assigned. *4N is a busy, post op orthopedic and spine floor consisting of 24 beds with a 12 bed overflow unit. As an LPN, you would take patient assignments, complete medication administration including pain management, teach/abide by specialty precautions, etc. Great opportunity as an LPN to gain more autonomy and further career if wanted. *What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following: * Two years of recent LPN experience preferred* State IV Certification upon hire or within three month of hire/transfer* Nursing skills and knowledge base* Graduate of an accredited School of Nursing required* Current Colorado LPN license* Current BLS from American Heart Association requiredPhysical Requirements-Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)Your Connected Community:When people and places align, incredible things happen.
