Overview
The Congestive Heart Failure (CHF) Coordinator provides oversight and coordination of all elements of the Heart Failure Program in collaboration with the leadership and the Medical Director. Responsibilities include the planning, organizing, and evaluation of patient care within the program as well as compliance with regulatory bodies and national guidelines for stroke management. This position reports to the Executive Director of Cardiac and Vascular Services.
Responsibilities
Initiates face to face collaborative, communication with nurses, attending/consulting physicians and other health care providers to discuss, confirm or make suggestions regarding patient management and evidence based standards. Evaluates patient care for compliance with evidence based standards of care by reviewing physician and nursing documentation and diagnostic studies.
Identifies opportunities for improvement and facilities related action plans no less than 3 per FY.
Assures that the daily needs of the CHF patient, including guidance w/ patient weight, I&O, patient discharge documentation requirements & use of order sets is greater than 95% per FY.
Develops a comprehensive individualized plan of care for patients, which includes teaching, discharge planning, and necessary referral to other members of the health care team in collaboration with healthcare team.
Takes accountability and assures communication of post discharge follow up phone calls/visits to assure transitions of care. Data to be presented by the 30th of the following month to CVSL ED and CHF MD.
Partner in the development and implementation of a hospital-wide program to decrease Heart Failure (HF) readmissions and excess post hospital ER visits to maintain hospital compliance with current CMS regulation.
Data to be presented by the 30th of the following month to CVSL ED and CHF MD. Develops measurable objectives to achieve outcome measures demonstrate the effectiveness of CHF Program and compliance with TJC Advanced Heart Failure disease specific certification standards.

Demonstrates the ability to compile and synthase data by completing reports (e.g. PI Dash Boards, benchmarks, etc.) by facilitating monthly Heart Failure committee meeting inclusive of all best practice CHF measurements of care. Meeting is presented in a succinct and professional manner.
Act as a clinical resource for members of the Beebe Healthcare team and community. Plans & collaborates the development of an annual community outreach event for public education of heart failure awareness to assure at least one event annually.
Utilizes appropriate teaching strategies, promotes an environment that facilitates learning and evaluates the effectiveness of patient/family teaching and collaborates with in the provision of education to nursing staff, hospital staff, patients, and families.
Assures organization is Heart Failure Joint Commission ready 365 days and prepared for certification review surveys.
Qualifications
Minimum 2 years of recent clinical experience directly related to cardiology. Experience in performance improvement principles and group facilitation / process redesign principles, presentations required.
Bachelor's degree in Nursing (BSN) - Required, or in pursuit of. Masters Degree - Preferred
Must document a minimum of 8 hours of Heart Failure specific education, and an additional 8 hours of other cardiovascular related education annually-Required.
BLS required.
CCRN and CHF certification by the Heart Failure Nursing Certification Board (CHFN) is preferred. Familiarity with (GWTG) Get With The Guidelines - Heart Failure American Heart Association - Preferred
